RESUMO

Volunteers in sports, full-time workers in sports, trainers, coaches, and administrative support people all have varying levels of devotion to their employment and organizations. Athletes are encouraged to participate in sports by their coaches, parents, and trainers. This helps athletes maintain physical fitness and promote a healthy lifestyle in order to encourage better international and local collaboration. A sports company is made up of championships, players, a management board, local cooperation, leagues, sports clubs, and men's and women's teams that supervise and conduct the organization's activities. Protecting the rights of sport's athletes is the responsibility of sports organizations, whether temporary or permanent. The purposive sample approach and snowball sampling were used in the research design to acquire data. Purposive sampling has helped researchers to find responders who are familiar with the field of expertise, based on the appropriate degree of knowledge necessary. A questionnaire was used to obtain the information. A total of fifty people took part in this study. Out of the 50 people who responded, 35 were men and 15 were women. SEM PLS 3.3.7v was used to evaluate the collected data. The findings revealed a substantial positive relationship between the factors.(AU)

RESUMO

BACKGROUND: Workplace wellness and disease-prevention programmes have been given increasing attention as a means to improve employee health and lower health costs. Health risk factors have been associated with improved work productivity. Further, health risk is reduced with improved cardiorespiratory fitness (CRF). Therefore, personal and workplace benefits may be achieved by workplace wellness programmes. OBJECTIVES: To investigate the effect of an on-site exercise-based wellness programme in a South African (SA) corporation. The impact of the programme on health risk factors, physiological parameters and corporate productivity indices was assessed. METHODS: Members joining the on-site gym facility of an SA corporation embarked on an individualised 12-week exercise programme, designed using the MyWellness Technogym Cloud platform and based on the participant's cardiovascular risk stratification. Weight, height, body mass index (BMI), blood pressure, waist circumference, CRF and muscle strength measures were assessed at the start and the end of the 12-week intervention. The number of pre- and postintervention cardiac risk factors were also applied to the Association of Health Productivity Management formula to estimate the effect of the intervention on absenteeism, presenteeism and productivity loss. RESULTS: The number of cardiovascular risk factors decreased in the low-, medium- and high-risk groups (68.1%, 42.7% and 41%, respectively). Body mass and BMI decreased significantly (mean (standard deviation (SD) ‒0.3 (5.0)% and ‒0.3 (5.4)%, respectively). Waist circumference also decreased significantly by 2.2% (6.3%). Only the diastolic blood pressure component of blood pressure changed significantly (‒1.2 (14.7)%) and VO2 peak increased by 14.2%. Upper- and lower-body strength improved by 17.9% and 20%, respectively. VO2 peak, BMI and lower-body strength were the only variables that changed significantly after effect size calculations were applied. Furthermore, the predicted impact on productivity loss showed a 1.1% improvement. This was the result of a predicted reduction in absenteeism (0.4%) and a reduction in presenteeism (0.8%). CONCLUSIONS: This corporate on-site exercise intervention programme positively affected the cardiovascular risk factors, biometrics, muscle strength and CRF of employees. These health outcomes decreased employee productivity loss. On-site workplace wellness programmes should be encouraged.

RESUMO

OBJECTIVES: The potential of sports clubs to promote health beyond physical activity has been acknowledged by researchers and policy-makers. This study gathered stakeholder ideas on support sports clubs need to increase health promotion efforts and prioritize them based on importance and feasibility. STUDY DESIGN: The study design used in this study is a mixed-methods concept mapping approach. METHODS: French sports and public health stakeholders (n = 45) were invited to participate. Steps included are as follows: (1) formulating a focus prompt, (2) brainstorming statements in response to the focus prompt, (3) sorting statements into themed piles, and (4) rating statements based on indicators. Multidimensional scaling and hierarchical cluster analysis were used to produce visual cluster maps, and descriptive statistics generated Go-Zone graphs based on mean importance and feasible ratings. RESULTS: Participants generated 62 statements from the focus prompt: 'What assistance would benefit sports clubs to become a health-promoting setting?'. Final sorting produced 9 clusters: Tools for health promotion, Communication tools, Stakeholder training courses, Diagnostic and Financing, Awareness and Mobilization, Advocacy, Policies and Methods, Sharing and Networking, as well as Communication and Dissemination. Participant ratings produced 34 statements within the Go-Zone graphs. CONCLUSION: Understanding stakeholders' needs to increase health promotion activities in sports clubs is crucial to planning and implementing sustainable health promotion policies and practice. Priority areas include increasing awareness of health promotion benefits, mobilizing actors, advocating for support, and educating sports club actors.

RESUMO

In line with the Sustainable Development Goals (SDGs) and the target for achieving Universal Health Coverage (UHC), state level initiatives to promote health with "no-one left behind" are underway in India. In Kerala, reforms under the flagship Aardram mission include upgradation of Primary Health Centres (PHCs) to Family Health Centres (FHCs, similar to the national model of health and wellness centres (HWCs)), with the proactive provision of a package of primary care services for the population in an administrative area. We report on a component of Aardram's monitoring and evaluation framework for primary health care, where tracer input, output, and outcome indicators were selected using a modified Delphi process and field tested. A conceptual framework and indicator inventory were developed drawing upon literature review and stakeholder consultations, followed by mapping of manual registers currently used in PHCs to identify sources of data and processes of monitoring. The indicator inventory was reduced to a list using a modified Delphi method, followed by facility-level field testing across three districts. The modified Delphi comprised 25 participants in two rounds, who brought the list down to 23 approved and 12 recommended indicators. Three types of challenges in monitoring indicators were identified: appropriateness of indicators relative to local use, lack of clarity or procedural differences among those doing the reporting, and validity of data. Further field-testing of indicators, as well as the revision or removal of some may be required to support ongoing health systems reform, learning, monitoring and evaluation.

RESUMO

In February 2018, the Indian Government announced Ayushman Bharat Program (ABP) with two components of (a) Health and Wellness Centres (HWCs), to deliver comprehensive primary health care (PHC) services to the entire population and (b) Pradhan Mantri Jan Arogya Yojana (PMJAY) for improving access to hospitalization services at secondary and tertiary level health facilities for bottom 40% of total population. The HWC component of ABP aims to upgrade and make 150,000 existing Government Primary health care facilities functional by December 2022. The first HWC was launched on 14 April 2018 and by 31 March 2020, a total 38,595 AB-HWCs were operational across India. This article documents and analyses the key design aspects of HWCs, against core components of PHC & the health system functions. The article reviews the progress and analyses the potential of HWCs to strengthen PHC services and therefore, advance Universal Health Coverage in India. Challenges emerged from COVID-19 pandemic & learnings thus far has also been analyzed to guide the scale up of HWCs in India. It has been argued that effectiveness and success of HWCs will be dependent upon a rapid transition from policy to accelerated implementation stage; focus on both supply and demand side interventions, dedicated and increased funding by both union and state governments; appropriate use of information and communication technology; engagement of community and civil society and other stakeholders, focus on effective and functional referral linkages; attention on public health services & population health interventions; sustained political will & monitoring and evaluation for the mid-term corrections, amongst other. Experience from India may have lessons and learnings for other low and middle-income countries to strengthen primary healthcare in journey towards universal health coverage.

RESUMO

OBJECTIVE: People with chronic and severe mental health issues die 15-25 years earlier than people in the general population - the main reason for this is poor physical health. An innovative treatment model for the management of primary care within a mental health service setting for 'hard to engage' patients is presented. CONCLUSION: The Wellness Clinic model provides the final level of care, where individuals who are at risk will receive a thorough physical health check and be referred for any additional tests and scans that are needed ensuring that any health issues are addressed. Ongoing monitoring and treatment occurs with the overall aim of reintegrating individuals back into the community through linking them in with community GPs.

RESUMO

BACKGROUND: Risky alcohol consumption is responsible for a variety of chronic and acute harms. Individuals involved in organised sport have been identified as one population group who consume risky amounts of alcohol both at the elite and the non-elite level. 'Good Sports', an alcohol management intervention focused on the community sports setting has been successful in addressing risky alcohol use and alcohol-related harm amongst players and sports fans. Sustaining such implementation effects is a common challenge across a variety of community settings. The primary aim of this trial was to assess the effectiveness of a web-based program in sustaining the implementation of best-practice alcohol management practices by community football clubs, relative to usual program care (i.e. control clubs). METHODS: Non-elite, community football clubs in the Australian states of New South Wales and Victoria, that were participating in an alcohol management program (Good Sports) were recruited for the study. Consenting clubs were randomised into intervention (N = 92) or control (N = 96) groups. A web-based sustainability intervention was delivered to intervention clubs over three consecutive Australian winter sports seasons (April-September 2015-2017). The intervention was designed to support continued (sustained) implementation of alcohol management practices at clubs consistent with the program. Control group clubs received usual support from the national Good Sports Program. Primary outcome data was collected through observational audits of club venues and grounds. RESULTS: A total of 92 intervention clubs (574 members) and 96 control clubs (612 members) were included in the final analysis. At follow-up, sustained implementation of alcohol management practices was high in both groups and there was no significant difference between intervention or control clubs at follow-up for both the proportion of clubs implementing 10 or more practices (OR 0.53, 95%CI 0.04-7.2; p = 0.63) or for the mean number of practices being implemented (mean difference 0.10, 95%CI -0.23-0.42; p = 0.55). There were also no significant differences between groups on measures of alcohol consumption by club members. CONCLUSIONS: The findings suggest that sustained implementation of alcohol management practices was high, and similar, between clubs receiving web-based implementation support or usual program support. RESUMO

In common with other countries in the World Health Organization South-East Asia Region, disease patterns in India have rapidly transitioned towards an increased burden of noncommunicable diseases. This epidemiological transition has been a major driver impelling a radical rethink of the structure of health care, especially with respect to the role, quality and capacity of primary health care. In addition to the Pradhan Mantri Jan Arogya Yojana insurance scheme, covering 40% of the poorest and most vulnerable individuals in the country for secondary and tertiary care, Ayushman Bharat is based on an ambitious programme of transforming India's 150 000 public peripheral health centres into health and wellness centres (HWCs) delivering universal, free comprehensive primary health care by the end of 2022. This transformation to facilities delivering high-quality, efficient, equitable and comprehensive care will involve paradigm shifts, not least in human resources to include a new cadre of mid-level health providers. The design of HWCs and the delivery of services build on the experiences and lessons learnt from the National Health Mission, India's flagship programme for strengthening health systems. Expanding the scope of these components to address the expanded service delivery package will require reorganization of work processes, including addressing the continuum of care across facility levels; moving from episodic pregnancy and delivery, newborn and immunization services to chronic care services; instituting screening and early treatment programmes; ensuring high-quality clinical services; and using information and communications technology for better reporting, focusing on health promotion and addressing health literacy in communities. Although there are major challenges ahead to meet these ambitious goals, it is important to capitalize on the current high level of political commitment accorded to comprehensive primary health care.

RESUMO

BACKGROUND: Physical inactivity is a pivotal factor in the development and progression of various chronic diseases. However, most fitness facilities exclude unhealthy individuals. Therefore, an exercise program that admits such patients is imperative. OBJECTIVES: To evaluate the effectiveness of a fitness facility that admits adult subjects with multiple chronic diseases. METHODS: We conducted a retrospective screening of patient records from the Medical Fitness Facility at Meir Medical Center, Israel. Intake of subjects was done by a multidisciplinary team. For each individual, personalized diet and exercise plans were developed and patients attended the facility twice a week. Each participant was evaluated at enrolment and after 4 months for well-being, metabolic parameters, exercise capacity, and laboratory blood tests. RESULTS: A total of 838 individuals were enrolled, mean age 57 years. Their medical conditions included dyslipidemia (48.8%), hypertension (37.6%), and diabetes mellitus (24.9%), followed by musculoskeletal problems (arthropathy 19%, lower back pain 16.1%) and ischemic heart disease (13.4%). Less common diagnoses were vascular diseases, pulmonary diseases, and malignancy. Only 40.5% of participants adhered to the regimen with advanced age being the best predictor for adherence. At the follow-up visit, body mass index was lower (31.2 vs. 30.2 kg/m2, P <0.0001), exercise capacity increased (measured as maximal MET; 7.1 vs. 8.1, P < 0.0001), and well-being improved (measured by Short Form Survey [SF-36]; 69.3 vs. 76.0, P <0.0001). CONCLUSIONS: We show that a fitness program for patients with multiple chronic diseases is feasible and effective in improving prognostic parameters, albeit significantly challenged by adherence limitations.

RESUMO

OBJECTIVE: To increase access to safe and appropriate exercise for people with balance and mobility limitations, community organizations have partnered with healthcare providers to deliver an evidence-based, task-oriented group exercise program in community centers in Canada. We aimed to understand challenges and solutions to implementing this program model to inform plans for expansion. RESULTS: At a 1-day meeting, 53 stakeholders (healthcare/recreation personnel, program participants/caregivers, researchers) identified challenges to program implementation that were captured by seven themes: Resources to deliver the exercise class (e.g., difficulty finding instructors with the skills to work with people with mobility limitations); Program marketing (e.g., to foster healthcare referrals); Transportation (e.g., particularly from rural areas); Program access (e.g., program full); Maintaining program integrity; Sustaining partnerships (i.e., with healthcare partners); and Funding (e.g., to deliver program or register). Stakeholders prioritized solutions to form an action plan. A survey of individuals supervising 28 programs revealed that people with stroke, acquired brain injury, multiple sclerosis, and Parkinson's disease register at 95-100% of centers. The most prevalent issues with program fidelity across centers were not requiring a minimum level of walking ability (32%), class sizes exceeding 12 (21%), and instructor-to-participant ratios exceeding 1:4 (19%). Findings provide considerations for program expansion.

Introduction: The National Academy of Medicine recently published "A Multifaceted Systems Approach to Addressing Stress Within Health Professions Education and Beyond," which calls for greater attention to the wellness of health care providers, students, and educators. Suggested actions include "creating and training positive role models." In order for faculty to role-model behaviors of wellness, they need to understand what this means and how to incorporate it into their lives. At present, there is only one other MedEdPORTAL resource on wellness and resilience that includes faculty as audience members. Methods: This 60-minute workshop is designed to provide faculty with a basic framework for wellness, with an emphasis on the emotional process aspect of resilience. Strategies for stress and anxiety management are reviewed. The workshop is interactive, with large- and small-group discussions, as well as a guided meditation. Results: This workshop was given at the 2017 Council of Faculties Business Meeting at the American Dental Education Association Annual Session, with 40 participants. Workshop evaluations, completed by 34 (85%) attendees, showed an overall rating of 4.0 on a 5-point Likert scale, with 5 = excellent. The workshop was repeated at the School of Dentistry at the University of California, San Francisco, with 11 attendees; there, the overall rating was 4.7. Discussion: In order to contribute to a culture of wellness and resilience in their organization, faculty need training on the concepts, as well as practical skills for implementation. This workshop provides introductory-level knowledge and skills.

RESUMO

Leadership development programs are common in sports, but seldom evaluated; hence, we have limited knowledge about what the participants actually learn and the impact these programs have on sports clubs' daily operations. The purpose of the current study was to integrate a transfer of training model with self-determination theory to understand predictors of learning and training transfer, following a leadership development program among organizational leaders in Swedish sports clubs. Bayesian multilevel path analysis showed that autonomous motivation and an autonomy-supportive implementation of the program positively predicted near transfer (i.e., immediately after the training program) and that perceiving an autonomy-supportive climate in the sports club positively predicted far transfer (i.e., 1 year after the training program). This study extends previous research by integrating a transfer of training model with self-determination theory and identified important motivational factors that predict near and far training transfer.

Introduction. Athletes are the principle consumers of nutritional supplements, and generally present the highest rate of usage. In Chile, there is a growing interest in consuming these supplements, although there is little knowledge of their effects. Objectives. To determine the consumer profile, who recommends the usage of these nutritional supplements, what the most used products are, and to distinguish possible differences in variables like sex, the purpose of their use, and others. Methods. A survey with 19 open and closed questions was given to 446 men and women volunteers, from between the ages of 17 and 49, who were participants from six gymnasiums in Santiago, Chile, in 2013. Results. Men are the greatest consumers of supplements, and usage increases according to the amount of time spent training every week. This group prefers protein shakes with the aim of increasing muscle mass. Women principally consume vitamins and lipolytic agents with the aim of reducing fatty mass. A high percentage believes that consuming supplements aids in meeting their goals. Conclusion. A considerable number of those who regularly attend a gymnasium consume supplements without specialized orientation and (possibly) without the real need of doing so. It is necessary to regulate irrational and potentially unsafe use of nutritional supplements in bettering physical output (AU)

Introduction: There are frequent changes in the supply of female child population, especially in aesthetic sports athletes by pressure to maintain a body. Objectives: The aim was to evaluate nutritional, anthropometric and weight perception aspects of girls who perform rhythmic gymnastics. Methods: 25 gymnast girls were compared to 25 control girls (control). Anthropometric assessment was done by ISAK protocol and nutrition through a seven-day dietary record; as well as a quantitative and qualitative assessment of dietary intake by the DIAL software. Nutritional assessment was performed using a seven days dietary record. A quantitative and qualitative assessment of food intake was conducted, using DIAL software. An ad hoc questionnaire to assess their perception of weight was completed. Results: Anthropometric data showed no significant differences between the study groups except for skinfolds and fat percentage, which was statistically lower in gymnast girls. Energy intake (1,413 ± 283 kcal/day) of gymnasts was lower than recommended by the Food and Nutrition Board, according to their age. Distribution of macronutrients was in the range of normal values (10-30% proteins, 45-65% carbohydrates, 20-35% fat). 32% of gymnasts and 36.4% of control girls felt they should weigh less. Conclusion: Considering the activity carried out by gymnasts, their energy requirements should conform to it, as this will contribute to their development and growth, and also to better perform their activity. Gymnasts’ food consumption is far from a balanced diet. No different behaviors were observed in the perception of weight between the two groups (AU)

The aim of this study was to verify the effect of a concurrent training program on the health of a woman of 42 years old with multiple lumbar herniated discs. The training consisted of functional, resisted and flexibility exercises over a period of 16 weeks. Body composition, strength, quality of life, flexibility and pain perception were analyzed. Improvements were detected in the percentage of fat, lean mass, fat mass, flexibility and leg strength. Pain perception score was zero after the 9th week in the cervical region and right hip. The same happened in the lower back, except that there was a slight increase in pain perception in the penultimate week, which was totally reduced at the end of the study. Physical and mental health, as well as total quality of life were improved. We conclude that the concurrent training improved health variables in the case study (AU)

RESUMO

School-based physical activity programs are only effective for increasing adolescents' school-based physical activity. To increase out-of-school-time physical activity, complementary community programs are warranted. Partnerships between universities and community organizations may help build the capacity of these organizations to provide sustainable programs. To understand capacity building processes and outcomes, we partnered with a YMCA to build on their adolescent physical activity promotion capacity. Together, we designed and implemented means to evaluate the YMCA teen program to inform program planning. For this qualitative case study, emails and interviews and meetings transcripts were collected over 2.5 years and analyzed using inductive and deductive thematic analysis. Findings illustrate that the YMCA's workforce and organizational development capacities (e.g., evaluation and health promotion capacity and competence) were increased through our partnership, resource allocation, and leadership. We responded to YMCA partners' perceived needs, yet guided them beyond those needs, successfully combining our complementary objectives, knowledge, and skills to generate an integrated program vision, rationale, and evaluation results. This provided YMCA partners with validation, reminders, and awareness. In turn, this contributed to programming and evaluation practice changes. In light of extant capacity building literature, we discuss how our partnership increased the YMCA's capacity to promote healthy adolescent programs.

RESUMO

This paper seeks to review the current international health-promoting sports club (HPSC) research, drawing together findings based on case studies from various countries to illustrate the status of HPSCs. In addition, future challenges for HPSC research and implementation are considered. The review includes six case studies from five countries. In summary, there are two major research themes in this area, namely 'research into HPSC activity' and 'research into HPSC networks'. The first theme investigates the extent to which sports clubs and/or national sports organisations invest in health promotion (HP) - both in policy and practice. The latter theme is driven by an intention to widen the scope of HPSCs to reach novel internal actors, like parents, siblings, etc., and/or external non-sporting bodies, like communities, schools, etc. The future challenges for HPSC research require a better understanding of the motives, barriers and capacities of sports clubs and coaches. Sports organisations, clubs and coaches generally support the intent of the HPSC concept, but even with the best evidence- or theory-based HP programmes/guidelines/standards, nothing will happen in practice if the nature and capacities of sports clubs are not better acknowledged. Therefore, a call for embracing implementation science is finally made to enhance implementation.
